Additional Instructions

  • All questions must be answered. If any question does not apply, please use N/A in that space. Do not leave the space blank.

  • Failure to answer questions or answers that are incomplete, untruthful, and/or lack required documentation will eliminate your application from consideration.

  • Application form and required documents must be submitted by March 15, 2025.

The Polish Heritage Association of Maryland (PHAM) awards scholarships yearly, which have ranged up to 2,000 dollars, to Maryland students of Polish descent pursuing post-secondary education. Award winners must attend the Scholarship Awards Reception in Baltimore on Sunday, May 18, 2025. Location information will be provided to awardees.

Please upload the following five (5) documents:

  • Two letters of recommendation: one from your current school and one personal recommendation.

  • Official or student copy of high school or post-secondary transcript.

  • Page one of your FAFSA student aid report that shows your FAFSA score and the student information.

  • An essay of up to 500 words, fully addressing each of the following areas. Do not exceed two double-spaced pages.

    - Tell us why you should receive a scholarship grant from PHAM.

    - Discuss the importance of your Polish heritage and its meaning to you.

    - Describe your plans for contributing to the Polish Community in the future.

Files must be in .doc or .pdf format. No image files, such as .jpg or .png

The completed PDF application may also be emailed to phaofmd@gmail.com with the required five (5) documents attached. Use the subject ‘PHAM Scholarship Application’ and send it by March 15, 2025. The file can be downloaded below.

Selection will be primarily based on financial need, with additional evaluation focusing on academic standing, leadership potential, and pride and participation in Polish heritage. The information provided in response to these questions will only be used in connection with this application. PHAM does not share this information with any outside parties.
